Scott,
Redact the person's name and identifying information then shred any copies (if any are made of the chart) after meeting. Also have everyone sign a confidentiality agreement that is kept in their educational files along with meeting minutes. The minutes should only mention the pertinent points and not any identifying information.
If age is an important factor - example a teen pregnancy - then you could leave the age of the patient - just not the birthday.
If it's under the guise of learning and teaching and improving then HIPPA only applies after the meeting when people talk about it with other people not present for the meeting.
We do this all the time with unique medical problems that come into our facility that most staff have not seen and don't know how to treat and with psychological autopsy's after deaths.

Greetings - a colleague sent me a note today for which I'm hoping the list
members can assist - She is wondering about other program's policies/etc.
regarding the review of charts (only their teams transport charts) by other
members of their transport team, i.e. in a peer review/case review setting
purely for purposes of education for their crew, especially on high risk. low
frequency type transports - Are there any privacy/legal concerns with this
practice? Any insight welcomed
