More list nesting bugs

As I told John a little while ago in a private email, there is an easy 
fix to all these nested-list problems. Instead on relying on the HTML 
block parser to convert lists and other blocks produced by Markdown 
into md5 hashes, the function that produce the list can output md5 
hashes directly.

This doesn't mean Markdown wouldn't benefit a better HTML block parser, 
but it would solve most problems we are currently facing.
