Attribute syntax for Markdown

I think ids are a great idea. I don't think it's a good solution for
footnotes, but I think it's a good idea for other reasons. For me too this
feature would eliminate 80% of the cases where I use HTML fragments. I would
add support for it to my [python markdown][1] if there were an agreement on
this.

Speaking of which, I ironed out most of the remaining issues, it seems,
except for the nested brackets (still looking for a non-ugly solution). In
particular, the link items get their <p> tags now as expected and links
within headers are supported. I also added support for footnotes and removed
xml.dom dependency.
