It looks promising, but I can't say I'm impressed with the output  
right now... I mean, given that input:

     ### test ###

     Hello world [link][1].

      [1]: http://test.com/

     [inline link](http://example.com)

I get this:

     <h3>
     test
     </h3><br /><br />

     Hello world <a href="http://example.com">link][1].<br /><br />

      [1]: http://test.com/<br /><br />

     [inline link</a>

Perhaps it'd be a good idea to use real paragraph tags. Once that's  
done, you'll be able to compare against the expected output for all  
the tests in [MDTest][].
