fenced code blocks.

Extra 1.2.1 (27 May 2008):

*	Fixed a problem where Markdown headers and horizontal rules were
	transformed into their HTML equivalent inside fenced code blocks.


In other words, this:

     ~~~
     # hello

     - - -
     ~~~

would generate a header and a horizontal rule *inside* the code block.
