Origin of the Wheel Report (CR-10)?

Here's a real needle-in-the-haystack question, but someone out there in List 
Land just might have the answer...

As a document for administering the whole "car record" process, when did the 
N&W introduce the CR-10 Conductor's Wheel Report?

Another way of approaching the question would be to ask, When was the "Car 
Record Office" established?  And before it was a "system" office, how were car 
records managed?

A pox on those who poke about in the dark Urgeschichte !  Away with all those 
who entertain such perfidious musings !
