Thanks Matt,
I ship packages to Canada quite often. I use the UPS web site and ship UPS. They ask a lot of questions on-line. Then you print out the completed forms and shipping label. Give yourself lots of time for the first one you send. Some questions require a little research. You're dealing with two countries, one the exporting country, and the other the importing country. Both has laws regarding export and import.

Hello friends -
If you haven't recently sent carvings or other items to Canada, you
may be surprised to learn that for security reasons, inbound
packages to Canada are now subject to stricter mail address and
import regulations. Packages not meeting the requirements will
apparently be returned to the sender for correction.
Regulations include, for example, that full first and last name of
the sender and recipient be included. The address must be printed in
ink or typewritten in CAPS only. The last line of the address must
show only the country; the postal code may also appear as the last
line of the address.
There is also a longer import form, that requires a specific
description of the item; "gift" or "present" is no longer acceptable.
