[Woodcarver] Sample Sales Contract/Invoice

Hi folks,
Anyone out there have a sample contract/invoice that one can use when offering carvings to galleries/gift shops for commission sales?  There's a new gift and craft store opening up near me and they are interested in some of my stuff. I just want to be prepared.
I remember reading about and even seeing such a sample on the web but have lost the URL and can't remember the site or author. It was at least a year ago. If someone knows of this site, please direct me. Thanks.
