REQUEST FOR PROPOSAL (RFP) FOR DEBRIS MANAGEMENT AND REMOVAL SERVICES

The St. Bernard Parish, herinafter called "Owner", has officially declared a
State of Emergency as a result of Hurricane Katrina. In order to deal with
the consequences that the landfall of this major storm event, and any other
event, has caused, the St. Bernard Parish Department of Public Works will
accept proposals from qualified contractors with experience in disaster and
debris removal services and the preparation, response, recovery, and
mitigation phases of any emergency situation or disaster. The Owner has
already completed the first phase of the debris removal process. All the
debris is currently being removed from the Parish rights of way, and is
stored at one (1) location within the Parish. As part of this contract the
contractor is responsible for the management and disposal of the materials
already stored on this site and the material being added daily. Also
included in the proposal is a price to bring the site back to its original
condition. For the purpose of this RFP, debris is any construction
materials, brush, vegetation and tree materials, and white goods.

This RFP is for the management of all eligible debris at the TDSRS including
following all Occupational Safety and Health Administration (OSHA) and
Environmental Protective Agency requirements to maintain a proper TDSRS.
This RFP is also for the picking up, hauling and disposal of the Debris
material and the white goods from each temporary debris storage and
reduction site (TDSRS). The disposal method and location should be outlined
in the proposal along with the name of the licensed contractor being used
for the recycling of the refrigerant. This proposal should outline how the
material will be picked up from each site (definition of a crew) and the
disposal location of the material. Two (2) towers approximately eight (8)
feet high with safety walls, roof and stairs needs to be provided large
enough to hold up to three (3) people safely at each TDSRS and final
landfill location. Additional towers may be erected to improve traffic flow
through the TDSRS. A quality assurance inspector, or inspectors, will be
located at this site to handle the haul tickets. A detail on how the tower
will be constructed or if lift equipment is chosen, the type of equipment
shall be included in this proposal. If scales are available at the disposal
site then the payment may be made by weight. The Parish reserves the right
to determine which method of payment is used.

All payments under the contract resulting from this Request for Proposal
(RFP) shall be made only for services requested and approved by the Owner.
The Owner holds the right to pay with in sixty (60) days from receipt of
approved invoice. No work effort will begin without written authorization
(Notice to Proceed) from the Owner or his designee. There shall be a 10%
retainer held from every invoice until the end of the project. The
successful contractor will be required to provide a $5,000,000 labor and
materials bond and a performance bond throughout the life of the project.
Additional items referenced in the fee schedule are considered as part of
the project scope. All work will be in general conformity with the
guidelines provided in FEMA Manual 325.
